基于数据挖掘分析穴位埋线治疗多囊卵巢综合征的选穴规律

Analysis of the Acupoint Selection Patterns of Acupoint Catgut Embedding Therapy in the Treatment of Polycystic Ovary Syndrome Based on Data Mining

目的：基于数据挖掘分析近20年穴位埋线治疗多囊卵巢综合征的选穴规律。方法：计算机检索中国知网期刊全文数据库（CNKI）、万方学术期刊全文数据库（Wanfang）、维普中文科技期刊数据库（VIP）、中国生物医学文献数据库（CBM） 中穴位埋线治疗多囊卵巢综合征的临床研究文献，检索时间为2005年4月1日—2025年4月1日。总结穴位埋线处方并建立数据库，运用SPSS26.0、SPSS Modeler18.0、Excel进行穴位频次统计，并行关联规则及聚类分析。结果：共纳入文献71篇，提取处方71组，共计48个穴位，腧穴引用频次累计662次。穴位埋线治疗多囊卵巢综合征使用频次前6位的穴位分别是关元、天枢、三阴交、丰隆、中脘及足三里；最常用的经脉为任脉及足阳明胃经；特定穴主要选取募穴及背俞穴，其次为五输穴、八会穴、下合穴、络穴、八脉交会穴、郄穴、原穴。治疗方法以穴位埋线联合中药为主。高频腧穴关联分析结果显示核心穴组为“天枢-丰隆-足三里”；聚类分析分为3类，第一类为脾俞、阴陵泉、水分、丰隆、足三里、三阴交、关元、中极、带脉；第二类为中脘、大横、水道；第三类为天枢、气海、子宫、血海。结论：穴位埋线在治疗多囊卵巢综合征中选穴多样化，主要以天枢、丰隆及特定腧穴为主穴，治疗思路多以通调脏腑为主。

Abstract:

Abstract： Objective： To analyze the acupoint selection patterns of acupoint catgut embedding therapy in the treatment of polycystic ovary syndrome （PCOS） over the past 20 years based on data mining. Methods： Clinical research literature on acupoint catgut embedding therapy for PCOS was retrieved from databases including China National Knowledge Infrastructure（CNKI），Wanfang Data Knowledge Service Platform （Wanfang），China Science and Technology Journal Database （VIP），and China Biology Medicine disc （CBM），with the search period from April 1， 2005 to April 1， 2025. Acupoint catgut embedding therapy prescriptions were summarized and a database was established. SPSS26.0， SPSS Modeler18.0， and Excel were used for acupoint frequency statistics， as well as association rule analysis and cluster analysis. Results： A total of 71 articles were included， yielding 71 acupoint prescriptions involving 48 acupoints， with a cumulative acupoint citation frequency of 662 times. The top 6 most frequently used acupoints in acupoint catgut embedding therapy for PCOS were Guanyuan （CV4），Tianshu （ST25）， Sanyinjiao （SP6），Fenglong （ST40），Zhongwan （CV12），and Zusanli （ST36）. The most commonly used meridians were the conception vessel and the stomach meridian of foot-yangming. Among specific acupoints，front-mu points and back-shu points were most frequently selected， followed by five-shu points， eight influential points， lower he-sea points， luo-connecting points， eight confluent points， xi-cleft points， and yuan-primary points. The main therapy was acupoint catgut embedding combined with Chinese medicine. Association rule analysis of high-frequency acupoints identified "Tianshu （ST25）- Fenglong （ST40）-Zusanli （ST36）" as the core acupoint pair. Cluster analysis divided the acupoints into three clusters： Cluster A included Pishu （BL20）， Yinlingquan （SP9）， Shuifen （CV9）， Fenglong （ST40），Zusanli （ST36），Sanyinjiao （SP6），Guanyuan （CV4），Zhongji （CV3），and Daimai （GB26）； Cluster B included Zhongwan（ CV12），Daheng（ SP15），and Shuidao（ ST28）；Cluster C included Tianshu（ ST25）， Qihai （CV6）， Zigong （EX-CA1）， and Xuehai （SP10）. Conclusion： The acupoint selection for acupoint catgut embedding therapy in the treatment of PCOS is diverse， primarily focusing on Tianshu （ST25）， Fenglong （ST40）， and specific acupoints. The therapeutic approach predominantly emphasizes regulating and harmonizing the zang-fu organs.
