从肺-肠轴论治过敏性哮喘

Treatment of Allergic Asthma from the Perspective of the Lung-Gut Axis

摘要:

肺-肠轴在免疫调控中的跨器官对话机制在呼吸免疫性疾病中发挥重要作用。多学科研究表明，肺-肠轴通过免疫、微生物及神经网络实现系统性调节：肺与大肠在胚胎发育上的同源性及黏膜免疫结构的相似性构建了功能协同的基础；肠道菌群在结构与功能上参与肺部免疫稳态的维持与调控。“肺与大肠相表里”理论为探索过敏性哮喘跨器官调控提供了新视角，单味中药或复方（如防风通圣散） 可通过多靶点干预肺-肠轴，调节肠道微生态并重塑免疫平衡，从而实现肺肠同治。这一理论框架不仅为中医肺肠同治提了科学解释，也为基于肠道微生态与免疫调控的哮喘干预策略开辟了新的研究方向。

Abstract：The cross-organ dialogue mechanism of the lung-gut axis in immune regulation plays an important role in respiratory immune diseases. Multidisciplinary studies have shown that the lung-gut axis achieves systemic regulation through immune， microbial， and neural networks： the embryonic development homology between the lung and the large intestine， along with the parallels in mucosal immune structures， forms the basis for functional coordination； while gut microbiota participates in maintaining and regulating pulmonary immune homeostasis through both structural and functional pathways. The traditional Chinese medicine theory of "the lung and large intestine being interiorexteriorly related" offers a new perspective for exploring cross-organ regulation in allergic asthma. Chinese herbal medicines and Chinese medicine compounds （e. g.， Fangfeng Tongsheng San） can intervene in the lung-gut axis through multiple targets，regulate the gut microecology，and reshape immune balance，thereby achieving simultaneous treatment of the lung and large intestine. This theoretical framework not only provides a scientific explanation for the traditional Chinese medicine approach of simultaneous treatment of lung and intestine，but also opens up new research directions for asthma intervention strategies based on gut microbiota and immune regulation.
