筋骨同调推拿法配合牵引治疗神经根型颈椎病临床研究

Clinical Study on Tendon-Bone Co-regulation Tuina Combined with Traction for Cervical Spondylotic Radiculopathy

目的：观察筋骨同调推拿法配合牵引治疗神经根型颈椎病的临床疗效。方法：选取2025年1—10月于长兴县中医院就诊的神经根型颈椎病患者80例，按随机数字表法分为对照组和试验组各40例。对照组施以颈椎牵引治疗，试验组在对照组基础上给予筋骨同调推拿法治疗。每天1次，10次为1个疗程，治疗2个疗程。评价2组临床疗效，比较2组治疗前后颈椎病临床评价量表（CASCS） 评分、Northwick Park 颈痛量表（NPQ） 评分、颈椎功能障碍指数量表（NDI） 评分及颈椎活动度，统计不良反应发生率。结果：试验组总有效率95.00% （38/40）， 高于对照组80.00% （32/40）（P＜0.05）。治疗后， 2 组CASCS 评分较治疗前升高（P＜0.05），试验组CASCS 评分高于对照组（P＜0.05）。治疗后，2 组NPQ 评分、NDI 评分较治疗前降低（P＜0.05），试验组NPQ评分、NDI评分低于对照组（P＜0.05）。治疗后，2组颈椎侧屈、伸屈、旋转角度较治疗前扩大（P＜0.05），试验组颈椎侧屈、伸屈、旋转角度大于对照组（P＜0.05）。2组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论：筋骨同调推拿法配合牵引治疗神经根型颈椎病患者能进一步改善病情，增加颈椎活动范围，效果满意，且安全性良好。

Abstract：Objective：To observe the clinical effect of tendon-bone co-regulation tuina combined with traction in the treatment of cervical spondylotic radiculopathy. Methods： A total of 80 patients with cervical spondylotic radiculopathy treated in Changxing Hospital of Traditional Chinese Medicine from January to October 2025 were enrolled and divided into the control group and the trial group by random number table method， with 40 cases in each group. The control group was treated with cervical traction， while the trial group received tendon-bone coregulation tuina on the basis of the control group. The treatment was performed once a day，10 times as one course，for a total of two courses. The clinical effects were evaluated； the scores of Clinical Assessment Scale for Cervical Spondylosis （CASCS）， Northwick Park Neck Pain Questionnaire （NPQ）， and Neck Disability Index （NDI）， and cervical range of motion were compared before and after treatment in the two groups； the incidence of adverse reactions was recorded. Results：The total effective rate was 95.00% （38/40） in the trial group，higher than that of 80.00% （32/40） in the control group （P＜0.05）. After treatment， the CASCS scores in both groups were increased when compared with those before treatment （P＜0.05），and the CASCS score in the trial group was higher than that in the control group （P＜0.05）. The NPQ and NDI scores in both groups were decreased after treatment （P＜0.05），and the scores in the trial group were lower than those in the control group （P＜0.05）. The angles of cervical lateral flexion， flexion-extension， and rotation in both groups were significantly enlarged after treatment （P＜0.05）， and the trial group had a larger range of motion than the control group （P＜0.05）. There was no significant difference in the incidence of adverse reactions between the two groups （P＞0.05）. Conclusion：Tendon-bone co-regulation tuina combined with traction can further improve the condition， increase the cervical range of motion in patients with cervical spondylotic radiculopathy，with satisfactory curative effect and good safety.
