    摘要:

    唐奇志教授认为浊毒困脾是糖尿病周围神经病变发病的核心病机,贯穿糖尿病周围神经病变发生发展的始终。根据脾胃功能失调逐步累及四肢经络、脉络的病机演变规律,可分为初期、进展期和后期3个阶段。初期病机为浊毒困脾,清阳不升,治以化浊祛毒、健脾升清;进展期病机为浊毒入络,血分郁热,治以清热凉血、化瘀通络;后期病机为脏腑虚衰,络损难复,治以益气扶正、养血通络。

    Abstract:

    Abstract: Professor TANG Qizhi proposes that turbid-toxin encumbering the spleen is the core pathogenesis of diabetic peripheral neuropathy (DPN),and it runs through the process of the onset and development of DPN. Based on the pathological evolution pattern in which dysfunction of the spleen and stomach gradually involves the meridians and collaterals of the limbs,this process can be divided into three stages:the initial stage,the progressive stage,and the advanced stage. In the initial stage,the pathogenesis is turbid-toxin encumbering the spleen with failure of clear yang to ascend,treatment should focus on eliminating turbidity and removing toxin,strengthening spleen and ascending clear yang;in the progressive stage,the pathogenesis is turbid-toxin entering the collaterals with heat stagnation in the blood aspect,treatment should focus on clearing heat and cooling blood,removing blood stasis and unblocking collaterals; in the advanced stage,the pathogenesis is deficiency and decline of the zang-fu organs with collateral damage that is difficult to repair, treatment should focus on boosting qi and supporting healthy qi, nourishing blood and dredging collaterals.
