自血穴位注射疗法治疗变应性鼻炎疗效的Meta 分析

Meta-Analysis of Efficacy of Autohemotherapy Acupoint Injection for Allergic Rhinitis

摘要:

目的：系统评价自血穴位注射疗法治疗变应性鼻炎（AR） 的临床疗效。方法：计算机检索PubMed、Cochrane Library、Web of Science、中国知网期刊全文数据库（CNKI）、维普中文科技期刊全文数据库（VIP）、万方数据知识服务平台（Wanfang）、中国生物医学文献数据库中有关自血穴位注射疗法治疗AR的研究，检索时间为自建库至2025年5月1日。由2名评价员分别对检索出的文献进行筛选并提取有效数据，采用RevMan 5.4软件对数据进行Meta分析。结果：共纳入27项研究，共计2 187例患者。Meta分析结果显示，自血穴位注射疗法［比值比（OR） =3.64，95%置信区间（95%CI）（2.25，5.89），P＜0.000 01］ 及自血穴位注射疗法与其他疗法的联合使用［OR=3.18，95%CI（2.34，4.32），P＜0.000 01］ 均可明显提高临床总有效率。使用自血穴位注射疗法在提高干扰素-γ （IFN-γ）［均数差（MD） =3.04，95%CI （2.04，4.04），P＜0.000 01］、白细胞介素-12 （IL-12）［MD=3.03，95%CI （1.70，4.35），P＜0.000 01］，改善患者鼻部症状中喷嚏［MD=-0.31，95%CI（-0.37，-0.26），P＜0.000 01］、流涕［MD=-0.51，95%CI（-0.57，-0.44），P＜0.000 01］、鼻塞［MD=-0.54， 95%CI （-0.69， -0.40）， P＜0.000 01］、鼻痒［MD=-0.41， 95%CI （-0.55， -0.28）， P＜0.000 01］、鼻黏膜肿胀［MD=-0.46， 95%CI （-0.64， -0.29）， P＜0.000 01］， 鼻结膜炎生活质量调查问卷（RQLQ） 评分中日常生活［MD=-1.24， 95%CI （-1.96， -0.52）， P=0.000 8］、睡眠［MD=-1.44，95%CI（-2.10，-0.78），P＜0.000 1］、非鼻/眼症状［MD=-1.90，95%CI（-3.41，-0.40），P=0.01］、行为问题［MD=-1.86， 95%CI （-2.24， -1.48）， P＜0.000 01］、鼻部症状［MD=-2.36， 95%CI （-2.97， -1.74）， P＜0.000 01］、情感反应［MD=-1.45， 95%CI （ -2.14， -0.76）， P＜0.000 1］、眼部症状［MD=-0.50，95%CI （-0.60， -0.40）， P＜0.000 01］ 方面均优于对照组。但在改善免疫球蛋白E 水平［MD=-58.10，95%CI （-123.56，7.36），P=0.08］、白细胞介素-4水平［MD=-0.87，95%CI （-1.74，0.00），P=0.05］ 方面，与对照组比较，差异无统计学意义。结论：自血穴位注射疗法与单纯的西医或者中医等常规治疗相比有效性更高，并且能明显改善患者的鼻部症状，提升患者生活质量，提高血清IFN-γ、IL-12水平，且安全性较高。

Abstract:

Abstract： Objective： To systematically evaluate the clinical efficacy of autohemotherapy acupoint injection for allergic rhinitis （AR）. Methods：Studies on autohemotherapy acupoint injection for AR were retrieved from PubMed， Cochrane Library，Web of Science，China National Knowledge Infrastructure（CNKI），China Science and Technology Journal Database （VIP）， Wanfang Data Knowledge Service Platform （Wanfang）， and China Biology Medicine disc （CBM） databases from inception of each database to May 1， 2025. Two reviewers independently screened the retrieved literature and extracted valid data. Meta-analysis was performed using RevMan 5.4 software. Results：A total of 27 studies involving 2 187 patients were included. Meta-analysis results showed that autohemotherapy acupoint injection alone [OR=3.64，95%CI （2.25，5.89），P＜0.000 01] and its combination with other therapies [OR=3.18， 95%CI （2.34，4.32），P＜0.000 01] significantly improved the total clinical effective rate. Autohemotherapy acupoint injection was superior to the control group in increasing interferon-γ （IFN-γ） [MD=3.04，95%CI （2.04，4.04），P＜ 0.000 01] and interleukin-12 （IL-12） [MD=3.03，95%CI （1.70，4.35），P＜0.000 01]；alleviating nasal symptoms including sneezing [MD=−0.31，95%CI （−0.37，−0.26），P＜0.000 01]，rhinorrhea [MD=−0.51，95%CI （−0.57， −0.44），P＜0.000 01]，nasal congestion [MD=−0.54，95%CI （−0.69，−0.40），P＜0.000 01]，nasal itching [MD= −0.41，95%CI （−0.55，−0.28），P＜0.000 01]，and nasal mucosal swelling [MD=−0.46，95%CI （−0.64，−0.29）， P＜0.000 01]；and improving Rhinoconjunctivitis Quality of Life Questionnaire （RQLQ） scores in daily activities [MD= −1.24，95%CI（ −1.96，−0.52），P=0.000 8]，sleep [MD=−1.44，95%CI（ −2.10，−0.78），P＜0.000 1]，non-nasal/ ocular symptoms [MD=−1.90，95%CI（ −3.41，−0.40），P=0.01]，behavioral problems [MD=−1.86，95%CI（ −2.24， −1.48），P＜0.000 01]，nasal symptoms [MD=−2.36，95%CI （−2.97，−1.74），P＜0.000 01]，emotional response [MD=−1.45，95%CI （−2.14，−0.76），P＜0.000 1]，and ocular symptoms [MD=−0.50，95%CI （−0.60，−0.40）， P＜0.000 01]. However，there were no statistically significant differences compared with the control group in improving immunoglobulin E levels [MD=−58.10，95%CI（ −123.56，7.36），P=0.08] or interleukin-4（ IL-4） levels [MD=−0.87， 95%CI （ − 1.74， 0.00）， P=0.05]. Conclusion： Autohemotherapy acupoint injection is more effective than conventional western medicine or conventional Chinese medicine alone，and significantly improves nasal symptoms and quality of life，increases serum IFN-γ and IL-12 levels，and has a favorable safety profile.
