Abstract: Atrial premature beats (APBs) can be classified under the categories of palpitations and fearful throbbing in traditional Chinese medicine (TCM). The etiology and pathogenesis are divided into three types: deficiency syndrome, excess syndrome, and deficiency interwoven with excess syndrome. Modern TCM practitioners have further refined the syndrome differentiation system. In TCM treatment of APBs,decoctions (classic formulas and modern self-prescribed formulas), Chinese patent medicines (e. g., Wenxin Granules and other commonly used medicines, as well as hospital preparations), and external therapies (acupuncture, acupoint application, etc.) are often used alone or in combination. All these approaches can effectively improve clinical symptoms and reduce the frequency of APB episodes with few adverse reactions. Among them,Wenxin Granules have been rated as an A-class drug for the APBs-relevant syndrome type. Various internal and external TCM therapies centered on syndrome differentiation and treatment can serve as important options for the clinical management of APBs,providing diversified diagnostic and treatment strategies. However,current TCM research still has limitations,such as unclear mechanisms of action,defects in clinical trial design,and an imperfect efficacy evaluation system. Future efforts should be focused on deepening theoretical research,innovating scientific research methods,improving the TCM diagnostic and treatment system,and promoting the standardization and high-quality development of TCM treatment for APBs.
