天芎定眩汤联合手指点穴治疗痰瘀阻窍型眩晕临床研究

Clinical Study on Tianxiong Dingxuan Decoction Combined with Finger Acupoint Pressing for Vertigo of Phlegm-Stasis Blocking the Orifices Type

目的：观察天芎定眩汤联合手指点穴治疗痰瘀阻窍型眩晕的临床疗效。方法：选取2024年6月— 2025年5月湛江市第二中医医院脑病科收治的痰瘀阻窍型眩晕患者84例，采用随机数字表法分为观察组和对 照组各42例。对照组给予西医常规治疗，观察组在对照组基础上加用天芎定眩汤联合手指点穴治疗，2组疗程 均为2周。评价2组临床疗效，比较2组治疗前后中医证候积分、椎动脉、基底动脉血流速度及血清超敏C-反 应蛋白（hs-CRP） 水平，并统计不良反应发生率。结果：观察组总有效率92.86% （39/42），高于对照组 76.19% （32/42）（P＜0.05）。治疗后，2组中医证候积分较均治疗前下降（P＜0.05），且观察组中医证候积分 低于对照组（P＜0.05）。治疗后，2组椎动脉、基底动脉的收缩期峰值流速（Vs）、舒张末期流速（Vd）、平均 流速（Vm） 均较治疗前加快（P＜0.05）；且观察组椎动脉、基底动脉的Vs、Vd、Vm 均快于对照组（P＜ 0.05）。治疗后，2组hs-CRP水平均较治疗前下降（P＜0.05），且观察组hs-CRP水平低于对照组（P＜0.05）。 2组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论：天芎定眩汤联合手指点穴治疗痰瘀阻窍型眩 晕疗效确切，可有效改善患者临床症状，提升脑血流灌注，减轻炎症反应，且操作简便、安全性高。

Abstract:

Abstract： Objective： To observe the clinical effect of the combination of Tianxiong Dingxuan Decoction and finger acupoint pressing on vertigo of phlegm-stasis blocking the orifices type. Methods： A total of 84 patients with vertigo of phlegm-stasis blocking the orifices type admitted to the Department of Encephalopathy， Zhanjiang Second Hospital of Traditional Chinese Medicine from June 2024 to May 2025 were selected and divided into the observation group and the control group by the random number table method，with 42 cases in each group. The control group was treated with routine western medicine， and the observation group was treated with Tianxiong Dingxuan Decoction combined with finger acupoint pressing based on the treatment in the control group. The course of treatment in both groups was two weeks. The clinical efficacy of the two groups was evaluated；the traditional Chinese medicine syndrome scores，the blood flow velocity of vertebral artery and basilar artery，and serum hypersensitive C-reactive protein （hs- CRP） levels before and after treatment were compared between the two groups；the incidence of adverse reactions was recorded. Results： The total effective rate was 92.86% （39/42） in the observation group， higher than that of 76.19% （32/42） in the control group （P＜0.05）. After treatment，the traditional Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment （P＜0.05）， and the above scores in the observation group were lower than those in the control group （P＜0.05）. After treatment， the peak systolic velocity （Vs）， end diastolic velocity （Vd） and mean velocity （Vm） of vertebral artery and basilar artery in the two groups were increased when compared with those before treatment （P＜0.05）；the Vs，Vd，and Vm of vertebral artery and basilar artery in the observation group were higher than those in the control group （P＜0.05）. After treatment，the hs-CRP levels in the two groups were decreased when compared with those before treatment （P＜0.05），and the hs- CRP level in the observation group was lower than that in the control group （P＜0.05）. There was no significant difference being found in the comparison of the incidence of adverse reactions between the two groups （P＞0.05）. Conclusion： The combination of Tianxiong Dingxuan Decoction and finger acupoint pressing has a definite curative effect on vertigo of phlegm-stasis blocking the orifices type，which can effectively improve the clinical symptoms and the cerebral blood perfusion of patients，and reduce the inflammatory responses，and is easy to operate and safe.
